Botox® works differently for migraines than it does for facial wrinkles. It is injected into specific muscles and areas around the head, neck and upper shoulders.

The treatment temporarily interrupts the release of chemicals involved in pain signalling. This may prevent some pain signals from reaching the nerve endings involved in migraine attacks.

Botox® does not treat a migraine that is already in progress. It is used as a preventive treatment, with the aim of reducing the number of headache and migraine days experienced each month.

The recognised chronic migraine protocol uses 155 units divided between 31 injection sites across seven areas of the head and neck. Your doctor will assess whether this protocol is appropriate for you.

What to Expect

Your appointment begins with a detailed medical consultation. Your doctor will review your symptoms, headache frequency, migraine history, medications, previous treatments and any other medical conditions.

You may be asked to complete a migraine questionnaire or provide a headache diary. This helps the doctor establish how many headache and migraine days you experience and whether your symptoms meet the criteria for chronic migraine.

If treatment is suitable, Botox® is administered through a series of small injections across the forehead, temples, back of the head, neck and upper shoulders. The injection stage is relatively quick, but additional time is required for consultation, assessment and preparation.

Preparing for Your Visit

Before Your Treatment

Keep a headache diary before your consultation if possible. Record the number of headache days, migraine symptoms, duration, possible triggers and medication taken. Bring details of previous migraine treatments and any relevant medical reports.

Tell your doctor about all medications and supplements, particularly muscle relaxants, injected antibiotics, blood-thinning medication and other migraine treatments. Do not stop prescribed medication unless instructed by the prescribing doctor.

You should also disclose previous botulinum toxin injections, allergies, neuromuscular conditions and any history of swallowing or breathing difficulties.

Botox® does not usually provide immediate migraine relief. Some patients notice an improvement within two to four weeks, while others may need two or three treatment cycles before the benefit can be assessed properly.

The response is measured by changes in headache days, migraine days, attack severity, medication use and the effect migraines have on your daily life.

Treatment does not permanently cure migraines, and results cannot be guaranteed. If it is effective, repeat treatment is generally considered every 12 weeks as part of an ongoing migraine-management plan.

After Your Appointment

Aftercare

Avoid rubbing or massaging the injection areas for the remainder of the day. Keep your head upright for several hours and avoid strenuous exercise, saunas, steam rooms, hot baths and excessive heat for approximately 24 hours.

Mild tenderness, bruising, a temporary headache or neck discomfort can occur. Continue recording your symptoms in a headache diary so your response can be reviewed accurately.

Contact the clinic promptly if you experience significant weakness, increasing pain, signs of infection or another unexpected reaction.

Important Considerations

Botox® is a preventive treatment for suitable adults with chronic migraines. It does not provide immediate relief from a migraine that has already started and is not intended for every type of headache.

Temporary tenderness, bruising, headache, neck stiffness or muscle weakness may occur after treatment. Your doctor will review your headache pattern, medication and medical history before recommending treatment.

Seek medical assessment for headaches that are new, suddenly severe or noticeably different from your usual migraine symptoms.

Is Botox® suitable for every type of migraine?

No. Botox® is specifically used to prevent chronic migraines in suitable adults. Its effectiveness has not been established for episodic migraines involving 14 or fewer headache days per month.

Can Botox® stop a migraine that has already started?

No. It is a preventive treatment and does not provide immediate relief during an active migraine attack. Continue using acute medication according to your prescribing doctor’s advice.

How many injections are used?

The recognised protocol uses 31 injection sites across seven areas of the head and neck. The recommended total dose is 155 units.

When might I notice an improvement?

Some patients notice changes within two to four weeks. Others require two or three treatment cycles before they can properly assess the benefit.

Will Botox® remove my forehead lines as well?

Some injection sites overlap with areas used for cosmetic Botox®, so facial movement or lines may change. However, migraine treatment follows a medical protocol rather than a cosmetic treatment plan.

Do I still need my migraine medication?

Possibly. Do not stop or change prescribed medication without speaking to your treating doctor. Botox® may form one part of a broader migraine-management plan.
